What’s your quad strength, bruh?

If you've had ACL surgery, heck, ANY knee surgery, you've probably been told a timeline — "you can return to sport around 9 months." That number is a helpful guideline, but it isn't the real answer to whether your knee is ready. What actually matters is what your quad muscle can do, and that's something we can measure.

Why the Quad Gets Weak After ACL Surgery

Almost everyone loses quad strength after ACL surgery — even people who feel like their knee is doing great. This happens for a few reasons: time in a brace, less walking and standing early on, and something called arthrogenic muscle inhibition. In simple terms, swelling and changes inside the knee send signals that make it harder for your brain to fully "turn on" the quad muscle, even when you're trying your hardest.

This matters because a weak quad changes how you move. When your quad can't do its job, your body finds a workaround — often by putting more stress on your hip, your other leg, or the knee itself during things like jumping, landing, or cutting. Over time, that workaround pattern is part of why weak quads are linked to a higher chance of re-injury.

The good news: this is fixable with the right strength work, and it's measurable, so you don't have to guess whether it's improving.

Two Numbers That Actually Tell You Something

When we test quad strength after ACL surgery, we look at two things together:

1. Limb Symmetry Index (LSI) — how your surgical leg compares to your other leg

2. Normalized strength — how strong your surgical leg is in absolute terms, adjusted for your body weight

Limb Symmetry Index (LSI)

LSI compares your surgical leg's strength to your uninvolved leg's strength, as a percentage:

LSI = (surgical leg ÷ other leg) × 100

If your surgical leg produces 80% of what your other leg produces, your LSI is 80%. This is useful because it's personalized to you — it's not comparing you to a stranger, it's comparing you to yourself.

But LSI has a catch: if your other leg has also gotten weaker during recovery (which happens more than people expect, especially if you've scaled back training overall), you could hit a "good" LSI number while both legs are actually weaker than they should be. That's why LSI alone isn't the full story.

Normalized Strength — the 3.0 Nm/kg Benchmark

This is where we bring in the second number. Normalized strength measures how much force your quad can produce relative to your body weight, expressed in Nm/kg (Newton-meters per kilogram). In the research on returning to sport after ACL surgery, a normalized quad strength of around 3.0 Nm/kg on your surgical leg is a commonly used benchmark — athletes who reach it tend to have better outcomes and lower rates of re-injury.

Put simply:

  • LSI answers: "Is my surgical leg catching up to my other leg?"

  • Normalized strength (3.0 Nm/kg) answers: "Is my surgical leg strong enough, period?"

You want both. It's possible to look good on one and still fall short on the other, which is exactly why we check both before clearing anyone for return to sport.

How This Gets Measured

Quad strength is tested using a handheld dynamometer (HHD) — a small device that measures how much force your leg produces during a maximal effort. The reading gets converted into torque, adjusted for your body weight, and compared side to side. This gives us an objective number instead of a guess, and lets us track your progress over time instead of only checking once at the very end.

Why This Is Worth Tracking, Not Just Testing Once

Strength testing shouldn't be a single pass/fail moment right before you return to sport — it should be checked along the way. Tracking your numbers throughout rehab shows whether you're on a good trajectory, catches plateaus early, and gives your care team time to adjust your program before you're standing at the finish line hoping you'll pass.

The Bottom Line

Time since surgery doesn't tell your knee's whole story — your quad strength does. Looking at both your Limb Symmetry Index and your normalized strength against the 3.0 Nm/kg benchmark gives you and your care team an honest, objective answer to the question that actually matters: is this leg ready to do what you're about to ask of it?
