“Just strengthen my glutes/Core”

Full disclosure: 15 years ago, I probably told you that. My bad.

If you've had low back or hip pain and sought help from a physical therapist, chiropractor, trainer, a well-meaning friend with (usually without) a kinesiology degree, OR CHATGPT, you've probably heard some version of: "Your glutes aren't firing" or "You need a stronger core."

It's become the default prescription — so ubiquitous that patients often arrive at their first visit already doing bridges and planks, having self-diagnosed a strength deficit before anyone's laid hands on them or watched them move.

The advice isn't wrong, exactly. It's incomplete, and its overuse reflects a persistent oversimplification of what actually generates and maintains pain.

The strength narrative is seductive because it's actionable

"Weak glutes" gives patients something concrete to fix. It's mechanistic, it's motivating, and it fits neatly into a rehab plan with sets and reps. The problem is that pain — especially persistent low back and hip pain — rarely has a single-variable explanation, and strength deficits are frequently a downstream finding rather than the driver.

A few things worth separating out:

Weakness on a dynamometer isn't the same as a motor control problem, and neither is the same as pain. I can find asymmetries in glute max or medius force output on plenty of pain-free people. Conversely, I regularly see patients with objectively adequate hip abductor strength who still can't tolerate single-leg loading without a flare. If the strength is there and the symptom persists, strengthening it further isn't the lever that moves the needle.

Muscle "activation" testing is a poor proxy for what happens during a real task. Manual muscle testing in supine tells you almost nothing about how someone organizes force through the pelvis during a step-down, a deadlift, or getting out of a car when they're guarding, fearful, or fatigued. The nervous system's strategy under load — timing, sequencing, co-contraction patterns — is a different problem than isolated muscle output, and it doesn't resolve with clamshells.

What's actually driving a lot of these presentations

Load tolerance and capacity mismatch, not absolute strength. Someone can have perfectly serviceable glute strength and still be in pain because their tissue's current capacity hasn't caught up to the demand they're placing on it — a training volume spike, a new activity, a life-stress-driven drop in recovery capacity.

The fix here is graded exposure and load management, not more isolated hip work stacked onto an already under-recovered system.

Central sensitization and threat perception. In patients with a longer pain history, hypermobility, or high pain catastrophizing scores, the nervous system's alarm threshold has shifted. Strengthening the glutes doesn't directly address a sensitized dorsal horn or a nocebo-laden belief that "my spine is unstable." These patients often get worse with aggressive strengthening protocols delivered without pain neuroscience education and graded, confidence-building exposure — not because the exercise is contraindicated, but because it's delivered in a context that confirms their fear of movement rather than dismantling it.

Movement variability, not a single "correct" pattern. A lot of "glute activation" cueing assumes there's one right way to hip hinge or one right way to load a step-down, and that deviation from it is pathological. The variability literature doesn't support that. Some people do better with more options in their movement repertoire, not a narrower, more "corrected" one. Chasing a specific firing pattern can actually reduce the adaptability that protects tissue under unpredictable real-world loading.

Joint-specific mechanical contributors that strength training doesn't touch. Hip impingement morphology, labral pathology, SI joint hypermobility with poor local force closure, or lumbar segmental hypermobility all present with "hip pain" or "back pain" that superficially looks like the textbook glute-weakness picture. Strengthening around a joint that's mechanically irritable from a structural or capsular issue can plateau quickly or aggravate symptoms, because the limiting factor isn't force production.

Psychosocial load. Sleep, stress, job dissatisfaction, and social support are consistently among the strongest predictors of chronicity in low back pain research — often stronger predictors than any biomechanical variable measured. A home exercise program aimed at glute strength doesn't touch any of that.

What a more accurate approach looks like

None of this means strength training is the wrong tool — it's frequently part of the plan. But it should be positioned as one lever among several, chosen because assessment supports it, not because it's the reflexive default.

In practice, that means:

  • Testing before prescribing. Objective capacity testing (force plates, HHD, functional load tolerance) to see whether strength is actually the limiting factor, versus assuming it based on symptom location.

  • Matching the intervention to the driver. Graded exposure and load management for a capacity mismatch; pain science education and confidence-building movement for a sensitized system; joint-specific mechanical assessment when the irritability pattern doesn't fit a simple strength story.

  • Widening the movement diet rather than narrowing it. Building tolerance across a range of positions and strategies instead of drilling one "correct" pattern.

  • Asking about sleep, stress, and life load as seriously as asking about squat depth.

  • Following irritability and response to loading over time, rather than committing to a single hypothesis (like "weak glutes") on day one and staying anchored to it regardless of how the patient responds.

The honest version of this advice is less satisfying than a poster on a gym wall: strengthening the glutes and core is sometimes the right call, for some patients, for some of their pain. Figuring out which patients, and why, is the actual clinical work — and it's a more useful use of a first visit than another round of clamshells.
