In Illinois, physical therapy does not require a referral/prescription from a MD. We are taught how to appropriately screen for diagnoses that require immediate attention or the care of another type of healthcare provider.

We offer out of network rates for evaluations and follow up appointments at $150/visit, due at time of service. We can provide you with a "superbill" (an itemized list of services) that you can then submit to your insurance provider to be applied towards an out of network deductible.

Medicare stipulates that if you seek an out of network provider for services, that you will pay out of pocket for theses services. You will not be able to submit your claims to Medicare OR your supplemental/secondary insurance.
